3D FIESTA与2D SE/FSE分级诊断膝关节前交叉韧带损伤的准确性分析

【摘要】 目的 以关节镜检查结果为对照,比较三维稳态采集快速成像序列(3D FIESTA)和常规序列在诊断前交叉韧带(ACL)损伤中的准确性。资料与方法 对50例患者的55个外伤膝关节行常规MRI(SET1WI,FSEPD/T2WI)和3D FIESTA序列扫描。依据MRI图像表现将ACL损伤分级,分析比较上述序列对ACL损伤诊断的敏感性、特异性、准确性和Kappa值。

The Diagnosis Research of Classification of the Anterior Cruciate Ligament Injury of Knee Joint by 3D FIESTA and 2D SE/FSE sequence

[Abstract] Objective Form the results of arthroscopy, the accuracy of three-dimensional fast imaging steady-state acquisition sequence (3D FIESTA) and conventional sequence (2D SE/FSE) in the diagnosis of the injury of anterior cruciate ligament is compared. Materials and Methods Do MRI (SET1WI, FSEPD/T2WI) and 3D FIESTA sequence scanning to 55 injured knees form 50 patients. Classifying the ACL injury based on MRI images, the sensitivity, specificity, accuracy and Kappa value of the above sequence to ACL injury in diagnostic are analyzed. Results The sensitivity, specificity, accuracy and Kappa values results of 3D FIESTA diagnosis of ACL injury are higher than those of conventional sequences (2D SE / FSE) ,the results are 94.6%, 77.8%, 89.1%, 0.745. Conclusion With short time of sequence image acquisition and high accuracy of ACL injury indicating, 3D FIESTA sequence is the supplementary of conventional sequence (2D SE / FSE) to knee examination.
前交叉韧带(anterior cruciate ligament,ACL)是维持膝关节稳定性的重要结构,是膝关节中最易发生损伤的韧带之一。随着关节韧带病变外科治疗手段的不断改进,临床对检测和准确判定韧带病变程度和治疗转归的要求更加迫切。MRI是一种公认的无创检测膝关节损伤的影像学方法,能够为ACL损伤提供诊断依据。近年来,一些针对ACL形态改变的特异MRI序列应用于临床。其中,三维稳态采集快速成像序列(3D FIESTA)由于可以明显缩短成像时间,近年来在实验研究和临床应用上受到广泛的关注[1-3]。本研究以关节镜为金标准,比较三维稳态采集快速成像(3DFIESTA)和常规序列(2DSE/FSE)诊断前交叉韧带(ACL)损伤的准确性,为临床应用提供更多依据。
